INDEPENDENT LIVING SKILLS TRAINING
PEER COUNSELING
OTHER COUNSELING Individuals with or without a disability providing assistance to consumers, family members, or significant others. Assistance is provided in the form of planning, decision making, conflict resolution, crisis intervention, etc.
HOUSING SERVICES To assist or arrange for a consumer securing a place to live or modifying an existing residence so that the dwelling is wheelchair accessible i.e. ramps. Referrals will be made to appropriate agencies when individual needs exceed wheelchair accessibility. Housing services do not include remodeling and/or major repairs.
EQUIPMENT LOAN DCAI has a supply of wheelchairs, commodes, walkers, crutches etc., that are loaned on a temporary basis at no charge until a resource can be found to supply the equipment needed.
TRANSPORTATION SERVICES To provide or arrange for a consumer's transportation needs in activities of daily living.
SOCIAL/RECREATIONAL SERVICES To assist a consumer in developing social skills and/or the productive use of leisure time, through a Center sponsored or coordinated program, or a community group, typically vocational in nature.
EDUCATIONAL SERVICES To secure teaching or tutoring services/supplies which could increase a consumer's proficiency in academic or vocational arenas.
VOCATIONAL SERVICES To provide a consumer with career counseling, job skill development, job placement, etc. In the majority of cases this will be done through referral to an agency such as Division of Vocational Rehabilitation.
COMMUNICATION SERVICES To assist a consumer in learning how to effectively communicate with others via sign language, braille, or other modes of alternative communications.
ATTENDANT/HOMEMAKER SERVICES To arrange for a consumer such services that can increase Independent Living potential through the aid of an attendant. The attendant will be managed by the consumer to assist the consumer with their activities of daily life.
FINANCIAL SERVICES Financial assistance is provided in emergency situations only for needs such as medication (when there are no other resources). Financial assistance does not include payment of medical bills, rent or house payments, car/truck payments or payments of any personal debts. All available resources from other agencies will be used to attempt to meet consumer needs.
EMERGENCY SERVICES To provide a consumer with assistance in obtaining food, heat, shelter, or similar supplies necessary to live in the event of an emergency situation. This does not typically involve direct monetary aid.
RESEARCH SERVICES To conduct research into any topic for a consumer in order to ensure that he/she has received the most currently available and appropriate information, training, or other services.
LEGAL SERVICES GUIDANCE To provide consumers with names of attorneys who will handle their problems at a low cost. Also to provide names of agencies and assist with completing forms, etc., required in order to get legal assistance.
BRAILLE TRANSCRIBING To provide computer generated Grade II Braille for the visually impaired consumer.
PAS PROGRAM OUTREACH Assist the DVR in program outreach for applicants who may be eligible to participate in the personal assistant services program.
PAS PROGRAM MANAGEMENT
PAS INQUIRES/PROBLEMS Process inquires pertaining to the PAS Program.
PAS TRAINING Skills training to employ and maintain attendants. These skills are those needed to instruct and supervise attendants and preparation of time sheets.
PAS EVALUATION Evaluation to assess the qualifications or participants in the program, this evaluation determines the consumers personal care needs.
PAS REEVALUATION Annual assessment of consumers needs and ability to manage the program.
PAS ATTENDANTS Maintain a list of people interested in being attendants and refer those individuals to consumers to screen, interview and employ.
